"Sample" letter for bank statement

Ola! Can somebody provide me with a "Sample" for the letter of proof from my german bank about my monthly pension. They need to know exactly what to write, and I'm going to germany to get my papers together! Any kind of help will be very much appriciated. You can also mail me offline! THANKS!
